Authorized Pick-up

The following individuals are authorized to pick up my child:

No other individual will be permitted to pick up the child unless authorized in advance by the parent/guardian.

Parent/Guardian Agreement

agree to the following terms and conditions for my child, 

to participate in the ABA therapy session at Autism Made Simple  hosted at Elgin West Community Centre and Pool.

Payment & Fees

• Pre-registration is required to secure the spot.
• The fee for the ABA session is $75 per hour., payable in full 2 days prior to the start of the session.
• "The ABA therapy fee is non-refundable and will not be reimbursed under any circumstances, including non-attendance due to illness, travel conflicts, or other reasons."

Drop-off and Pick-up Policy
This is a drop-off and pick-up program. Parents/guardians are not permitted to stay with their children in therapy sessions.
